What are Medicare Savings Programs?

Medicare Savings Programs (MSPs) are state-run programs that help people with Medicare pay their out-of-pocket costs. Depending on your income, an MSP can:

  • Pay your monthly Medicare Part B premium ($202.90 in 2026)
  • Cover Medicare deductibles, copays, and coinsurance (for people with the lowest incomes)
  • Automatically enroll you in the Extra Help program, which lowers your prescription costs

If you qualify, that’s about $200 back in your pocket each month.

Premium: The amount you pay each month to have Medicare coverage, whether or not you use it. For most people, the Part B premium is taken directly out of their Social Security check.

Deductible: The amount you pay out of pocket for health care each year before Medicare starts to pay. In 2026, the Part B deductible is $283.

Copay: A set dollar amount you pay for a service, like $20 for a doctor’s visit or a prescription.

Coinsurance: Your share of the cost of a service, figured as a percentage. After you meet your deductible, Medicare usually pays 80% of the cost of doctor visits and outpatient care, and you pay the other 20%.

Extra Help: A federal program that lowers what you pay for prescriptions under Medicare Part D, including premiums and copays.

More information is available from the Centers for Medicare & Medicaid Services.
